How to apply
| • |
Ask for Application form* at Central Credit Card Application booths, Information counters and Payment spots at every Central store and ZEN. |

Complete the application form (clear and easy-to-read handwriting) |

Affix your signature similar to the one given in the credit card application. |

Attach the photocopy of your previous month utility bill or receipt. |

Send the application form together with supporting document(s) mentioned in number 4 to officers at Central Credit Card Application Booths, Customer Services Counters at every Central store and Zen or send the form via mail. |

Monthly service fee/utility charge shall become effective the service providers have been notified of such approval as evidenced by the appearance of the cardholders credit card number on the utilities billing statement. |
Remark:
*Either primary or supplementary cardholder can apply for this product.
*This service is reserved only for individuals.
*Central reserves the right to limit monthly service fee charged by the Metropolitan Electricity Authority, the Provincial Electricity Authority at the maximum of Baht 30,000 per transaction.
